Key Responsibilities

• Conduct outpatient evaluations and develop individualized treatment plans for injuries across orthopedic and general physical therapy cases
• Provide 60 minutes of direct patient care per visit, scheduling patients every 30 minutes; manage approximately 11 visits per day and 55 visits per week
• Perform fall risk assessments and gross motor assessments across adult, adolescent, and geriatric populations
• Manage patient caseload including screening, caseload building, and ongoing case management
• Treat a broad range of orthopedic conditions including ACL tears, rotator cuff repairs, total joint replacements, disc herniations, osteoarthritis, plantar fasciitis, and more
• Document patient progress and treatment plans using NextGen EMR

Required Qualifications

• Active Physical Therapist license in the state of Illinois
• Minimum of 1 year of physical therapy experience
• Experience treating adult, adolescent, and geriatric patient populations
• Ability to work both morning and evening hours as needed
• Proficiency or ability to learn NextGen EMR

Preferred Experience / Skills

• Experience in an outpatient orthopedic clinic setting
• Familiarity with post-operative care including rotator cuff repair, ACL reconstruction, and total joint replacement
• Experience with case management and caseload building in a high-volume outpatient environment
