Key Responsibilities

  • Complete and maintain accurate OASIS documentation in compliance with agency policies and State and Federal regulations, ensuring timeliness standards are met.
  • Utilize the nursing process to assess patient and caregiver needs, develop individualized plans of care, and implement nursing interventions in accordance with evidence-based practice and applicable regulations.
  • Serve as Case Manager for an assigned patient caseload, coordinating care with physicians, providers, LPNs, and interdisciplinary team members to ensure safe and effective patient outcomes.
  • Maintain proficiency in high-tech clinical skills including IV use and maintenance, wound VAC, and complex wound care, while upholding infection control standards throughout the plan of care.
  • Perform patient and caregiver education on topics including medication side effects, treatment options, discharge instructions, and reportable conditions.
  • Coordinate patient discharges and transfers, participate in monthly peer review audits, and facilitate case conferences to support team communication and quality of care.

Required Qualifications

  • Active Registered Nurse (RN) licensure in the state of Vermont.
  • Experience in home health nursing, including OASIS documentation knowledge.
  • Ability to provide on-call coverage, including hospice support as needed.
  • Proficiency in case management and care coordination across interdisciplinary teams.
  • Strong clinical documentation skills and ability to complete State, Federal, and agency-required forms accurately and on time.

Preferred Experience / Skills

  • Experience with McKesson or similar home health electronic medical record systems.
  • Background in hospice or palliative care nursing.
  • Familiarity with Medicare home health regulations and notices such as HHABN and Medicare Notice of Non-Coverage.
