Key Responsibilities
- Provide speech-language pathology services to patients in a skilled nursing facility (SNF) setting
- Evaluate, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders
- Develop and implement individualized treatment plans based on patient needs and goals
- Document patient progress and maintain accurate clinical records using electronic documentation systems
- Collaborate with interdisciplinary care teams to ensure comprehensive patient care
Required Qualifications
- Active California Speech Language Pathologist (SLP) license
- Minimum of 1 year of experience in a skilled nursing facility (SNF) setting
- Proficiency with Rehab Optima (Net Health) documentation software
- Master's degree in Speech-Language Pathology or Communication Disorders
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred
Preferred Experience / Skills
- Experience with dysphagia evaluation and treatment in a post-acute or SNF environment
- Strong familiarity with Medicare and skilled nursing documentation requirements
