Key Responsibilities

• Provide direct nursing care to residents in a long-term care / skilled nursing facility setting
• Manage a patient population with diagnoses including dementia, Alzheimer's, diabetes, and CHF
• Maintain accurate and timely documentation using the Matrix EMR system
• Adhere to rotating weekend and holiday scheduling requirements
• Participate in on-campus facility orientation prior to beginning patient care assignments

Required Qualifications

• Active and unrestricted LPN or LVN license valid in the state of South Dakota
• Experience in a long-term care or skilled nursing facility (LTC/SNF) setting
• Ability to work night shifts on a 12-hour schedule
• Willingness to work rotating weekends and holidays
• Familiarity with or ability to learn the Matrix EMR system

Preferred Experience / Skills

• Prior experience caring for patients with dementia, Alzheimer's, diabetes, or CHF
• Previous experience using Matrix EMR
