Therapist Social Worker

Job Title
• Social Worker – Hospice

Location
• Albany, OR

Schedule / Job Type
• Full-time, day shifts, 3 shifts per week

Key Responsibilities
• Provide grief and loss counseling to patients and families in hospice settings
• Support patients in their final stages of illness through social services and emotional support
• Collaborate with interdisciplinary team to develop personalized care plans
• Offer emotional and spiritual support to patients and their families
• Document patient interactions and care plans accurately and timely
• Assist with resource coordination and community referrals as needed

Required Qualifications
• Bachelor's degree in social services, counseling, psychology, or related field
• Valid driver's license with auto liability insurance
• Recent experience in hospice or related healthcare setting preferred
• Experience or education in grief or loss counseling required
• Healthcare Provider Level BLS certification required

Preferred Experience / Skills
• Master’s degree in a relevant field preferred
• Additional experience in medical or community health settings
