Key Responsibilities

  • Monitor and amend patient visit schedules to maximize productivity and meet patient needs within Rutland and Bennington County.
  • Perform comprehensive assessments, including OASIS and PT evaluations, ensuring compliance with federal, state, organizational, and APTA standards.
  • Develop and implement individualized plans of care to promote functional independence and safety, setting patient-specific goals.
  • Provide skilled physical therapy treatments, including therapeutic techniques and patient/caregiver education, with documentation of progress and quality of care.
  • Coordinate patient care with interdisciplinary team members, including physicians, nurses, and rehab staff, maintaining effective communication.
  • Supervise and direct clinical activities of PTAs, PT students, and home care aides in accordance with regulatory standards.
  • Complete all required paperwork for documentation, billing, insurance, and regulatory compliance in a timely and accurate manner.
  • Participate in in-service training and community outreach activities to promote rehabilitation services.
  • Conduct patient health assessments and procedures such as wound care, staple/suture removal, and vital sign checks.
  • Communicate schedule changes and ensure timely documentation in compliance with agency, state, and federal guidelines.
  • Assist with patient discharges or transfers, ensuring all documentation is complete and compliant.
  • Report patient incidents (infections, injuries, etc.) promptly and participate in peer review audits.

Required Qualifications

  • Valid PT license in the state of Vermont.
  • Proven experience in outpatient or home health physical therapy.
  • Knowledge of federal and state regulations related to healthcare and therapy documentation.
  • Strong communication and teamwork skills.
  • Ability to perform clinical assessments and document findings accurately.

Preferred Experience / Skills

  • Experience with OASIS documentation.
  • Familiarity with Medicare and insurance billing protocols.
  • Experience supervising PTA or therapy students.
  • Knowledge of wound care and other patient-related procedures.
