Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based (Home Health)

Key Responsibilities

• Provide skilled physical therapy assistant services to patients in a home health setting across the west Portland area and surrounding communities
• Visit up to 6 patients per day, meeting a standard productivity expectation of 30 points per day
• Document patient care and clinical notes using EPIC Remote Client (laptop) and Rover mobile application
• Manage daily patient schedule independently, utilizing personal transportation to travel between patient homes with occasional drives of up to 60 miles per day
• Collaborate with the supervising physical therapist and interdisciplinary team to support patient care plans and goals

Required Qualifications

• Active Physical Therapy Assistant (PTA) license (pending license considered)
• Minimum of 1 year of skilled home health experience preferred; first-time home health travelers will be considered
• Basic Cardiac Life Support (BCLS) certification required
• COVID-19 primary vaccination series plus one booster required
• Reliable personal transportation required for daily patient visits
• Experience with OASIS documentation in a home health setting

Preferred Experience / Skills

• Familiarity with EPIC Remote Client or similar electronic health record systems used in home health environments
• Experience meeting productivity point expectations in a home health or field-based therapy role
• Ability to manage time effectively across a geographically distributed patient caseload
