Contract | Full-Time | Days (5x8-hour shifts, 8:00 AM – 4:30 PM) | Onsite / Field-Based
Key Responsibilities
- Provide skilled occupational therapy assistant services to patients in a home health setting across the west Portland area and surrounding communities
- Visit up to 6 patients per day, meeting a standard productivity expectation of 30 points per day
- Complete clinical documentation using EPIC Remote Client and Rover mobile application, including OASIS assessments
- Manage daily patient schedule independently, utilizing personal transportation to travel between patient homes with an occasional maximum radius of approximately 60 miles per day
- Collaborate with the supervising Occupational Therapist to implement and progress individualized treatment plans
Required Qualifications
- Active Oregon COTA license required
- Minimum of 1 year of skilled home health experience
- Current Basic Cardiac Life Support (BCLS) certification
- COVID-19 primary vaccination series plus at least one booster required
- Reliable personal transportation required for daily patient visits
- Experience with OASIS documentation
Preferred Experience / Skills
- Familiarity with EPIC Remote Client for home health charting
- Comfort working independently in a community-based, field-driven environment
- Experience meeting productivity point expectations in a home health setting
