Contract | Travel | Full-Time | Days | Monday–Friday | 8-Hour Shifts | 40 Hours per Week

Key Responsibilities

• Provide physical therapy services to patients in a home health setting across an assigned regional territory
• Conduct patient evaluations and revisits, with a typical daily caseload of 6 revisits (including at least one evaluation) or 4–5 revisits plus one admission
• Complete all clinical documentation within the standard 8-hour workday in compliance with productivity expectations
• Perform OASIS assessments as required for home health patients
• Work primarily within one assigned region while remaining available to cover outside of region as needed

Required Qualifications

• Active Delaware Physical Therapist license (or ability to obtain prior to start)
• Experience in home health physical therapy
• Proficiency with OASIS documentation
• Ability to complete in-person fingerprinting in Delaware as part of the credentialing process (allow approximately 5 weeks for non-resident credentialing)

Preferred Experience / Skills

• Familiarity with Sussex County, DE communities and surrounding regions
• Experience managing a high-volume home health caseload efficiently within productivity standards
