Contract | Full-Time | Onsite | Days | Monday–Friday, 8-hour shifts (40 hrs/week) | 13-Week Assignment

Key Responsibilities

• Provide skilled home health nursing care to patients across the assigned coverage area in lower New Castle County and Kent County, Delaware
• Perform OASIS assessments and documentation in accordance with home health standards and regulatory requirements
• Participate in on-call rotation approximately two times per month (maximum three times per month), responding to true clinical issues during designated on-call hours
• Conduct phone triage during on-call shifts and perform patient home visits when clinically necessary during applicable hours
• Participate in a weekend rotation of one weekend every four weeks, with scheduling flexibility available

Required Qualifications

• Active Delaware Registered Nurse (RN) license required
• Prior home health nursing experience required
• Proficiency with OASIS assessments required
• Ability to travel within the assigned coverage area (up to 60-mile radius)
• Must be available to complete in-person credentialing requirements in Delaware, including in-person fingerprinting; candidates should plan approximately five weeks for the full credentialing process

Preferred Experience / Skills

• Experience working in a home health environment across a multi-county coverage area
• Comfort with phone triage and independent clinical decision-making during on-call hours
