Contract | Full-Time | Days | Monday–Friday | 8-Hour Shifts | 40 Hours per Week

Key Responsibilities

• Provide physical therapy services to patients in a home health setting across an assigned regional coverage area in southern Delaware
• Conduct patient evaluations and revisits, with a typical daily caseload of 6 revisits (including approximately 1 evaluation) or 4–5 revisits plus 1 admission
• Complete all clinical documentation within the standard 8-hour workday
• Perform OASIS assessments in accordance with home health requirements
• Work primarily within one assigned region, with occasional coverage outside of the primary region as needed

Required Qualifications

• Active Delaware Physical Therapist license (or ability to obtain prior to start)
• Experience in home health physical therapy
• Proficiency with OASIS documentation
• Ability to meet productivity expectations within an 8-hour workday, including timely completion of documentation
• Willingness to complete in-person fingerprinting in Delaware as part of the credentialing process (non-resident applicants should allow approximately 5 weeks for credentialing)

Preferred Experience / Skills

• Prior travel therapy experience
• Familiarity with the southern Delaware region and surrounding communities
