Key Responsibilities

• Provide speech-language pathology rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF)
• Perform evaluations, reassessments, treatments, and other clinical services consistent with licensure and scope of practice
• Develop and implement individualized treatment plans in collaboration with the supervising therapist, physician, and interdisciplinary team
• Apply evidence-based therapeutic interventions to maximize communication, swallowing, function, and quality of life
• Educate residents, families, and caregivers regarding treatment plans and patient progress
• Complete accurate, timely, and compliant documentation in accordance with facility, state, federal, and payer requirements

Required Qualifications

• Current, active State of Hawaii Speech Language Pathologist license in good standing (must be obtained prior to assignment start date)
• Current Basic Life Support (BLS) certification
• Ability to work Hawaii State and Federal Holidays as required based on facility operations
• Willingness to float to other units or facilities based on census and staffing needs

Preferred Experience / Skills

• Minimum two (2) years of experience in a SNF or ICF setting
• Familiarity with EMR documentation systems such as Net Health Rehab Optima or Epic
