Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient or clinic setting
• Evaluate patient conditions and develop individualized treatment plans
• Implement therapeutic interventions and monitor patient progress toward goals
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with interdisciplinary team members to coordinate patient care

Required Qualifications

• Active Physical Therapist license in the state of Arizona (permanent license required)
• Graduate of an accredited Physical Therapy program
• Prior experience in a relevant clinical setting
• Ability to meet facility-specific credentialing and documentation requirements prior to start
• Must provide proof of license application within 5 business days of booking if licensure is pending

Preferred Experience / Skills

• Previous experience in an outpatient physical therapy environment
• Familiarity with electronic medical records and clinical documentation systems
