Key Responsibilities
- Manage an independent hospice patient caseload in a home health setting
- Provide compassionate, patient-centered care aligned with hospice care principles
- Coordinate and collaborate with an interdisciplinary team to ensure comprehensive patient care
- Conduct case management activities including assessments, care planning, and documentation
- Utilize electronic health record systems to maintain accurate and timely patient records
Required Qualifications
- Active and clear Connecticut RN license (or valid compact license)
- Associate's or Bachelor's degree in Nursing
- Prior hospice nursing experience
- Experience with Homecare Homebase (HCHB)
- Strong case management and interdisciplinary team collaboration skills
- Ability to independently manage a hospice caseload while delivering compassionate, patient-centered care
Preferred Experience / Skills
- Experience working in a home health or community-based hospice environment
- Familiarity with hospice regulatory requirements and documentation standards
