Key Responsibilities

• Lead all palliative care program startup activities, including development of workflows, policies, and operational design
• Supervise and support palliative care staff in collaboration with the Medical Director
• Oversee compliance with Medicare, Medicaid, commercial payers, and accreditation standards
• Manage scheduling, referrals, documentation, and care transitions to ensure operational efficiency
• Deliver direct social work services during program startup and on an as-needed basis
• Participate in the Manager On-Call rotation for hospice services

Required Qualifications

• Master's degree in Social Work (MSW) required
• Active Licensed Master Social Worker (LMSW) credential required
• Experience with Electronic Medical Records and Microsoft Office products
• Strong interpersonal, communication, and leadership skills with the ability to collaborate across disciplines
• Demonstrated ability to guide teams through periods of growth and organizational change

Preferred Experience / Skills

• Prior experience in hospice or palliative care leadership
• Prior program development or program launch experience
• Familiarity with provider credentialing and payer contracting processes
