Full-Time | Direct Hire | Onsite with field-based travel | Rotating On-Call
Key Responsibilities
- Lead all palliative care program startup activities, including development of workflows, policies, and operational design
- Supervise and support palliative care staff in collaboration with the Medical Director
- Oversee compliance with Medicare, Medicaid, commercial payers, and accreditation standards
- Manage scheduling, referrals, documentation, and care transitions to ensure operational efficiency
- Deliver direct social work services during program startup and on an as-needed basis
- Participate in the Manager On-Call rotation for hospice services
Required Qualifications
- Master's degree in Social Work (MSW) required
- Active Licensed Master Social Worker (LMSW) credential required
- Experience with Electronic Medical Records and Microsoft Office products
- Strong interpersonal, communication, and leadership skills with the ability to collaborate across disciplines
- Demonstrated ability to guide teams through periods of growth and organizational change
Preferred Experience / Skills
- Prior experience in hospice or palliative care leadership
- Prior program development or program launch experience
- Familiarity with provider credentialing and payer contracting processes
