Role Details

• Compensation: $50.00 - $65.00 per hour
• Benefits: Medical, Dental, and Vision
• Duration: Full-Time, Permanent
• Location: Phoenix, AZ

What You’ll Be Doing

• Develop and implement individualized treatment plans based on patient needs and physician guidance.
• Assess patient conditions, evaluate progress, and modify care plans to meet therapy goals.
• Collaborate with interdisciplinary team members to assess and plan patient care.
• Set and track realistic, achievable goals for residents’ functional improvement.
• Document patient assessments, treatment progress, and outcomes in compliance with regulations.
• Educate patients and families on therapy methods and ongoing care strategies.
• Oversee and participate in screening residents to optimize functional abilities.
• Communicate patient status, needs, and progress effectively with families and healthcare team members.
• Maintain punctuality and consistency to ensure quality patient care.

What We’re Looking For

• Education: Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an accredited program.
• Licensure: Current Physical Therapist license in the state of employment.
• Certifications: CPR certification required.
• Experience: Minimum of six months in a long-term care environment preferred.
