Contract (13 Weeks) | Days, 5x8 Hours (7:30–16:00) | Every Other Weekend Required | 72 Guaranteed Hours per 2-Week Period

Key Responsibilities

• Provide social work services across all areas of the hospital, including floating between two nearby campuses (less than 2 miles apart)
• Perform discharge planning and care coordination for a diverse patient population
• Support patients across multiple units including Med/Surg, long length of stay, and emergency department settings
• Collaborate with interdisciplinary teams to assess patient needs and develop appropriate care plans
• Document clinical interactions and care coordination activities using EPIC
• Commit to working 3 of 5 designated holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day

Required Qualifications

• Master of Social Work (MSW) degree required
• Active Washington State Social Work License required at time of submission — pending licensure not accepted
• BLS certification required
• Minimum 1 year of social work experience required
• Acute care experience required; discharge planning and care coordination experience required
• Must live more than 50 miles from the facility to be eligible as a traveler

Preferred Experience / Skills

• Diverse case management experience across Med/Surg, long length of stay (LLOS), and emergency department settings
• Experience working in a Level II Trauma Center or similarly complex acute care environment
• Familiarity with EPIC electronic health records system
