Contract | Days | Full-Time | 5x8-Hour Shifts (8:00–16:30) | Onsite
Key Responsibilities
- Provide skilled physical therapy services to home health patients in Seaside, OR and surrounding areas
- Manage a caseload of up to 6 patients per day with a standard productivity expectation of 30 points
- Complete OASIS documentation as required for home health compliance
- Chart using EPIC Remote Client and Rover mobile application
- Travel independently to patient homes, with occasional daily driving radius of up to approximately 60 miles
Required Qualifications
- Active Physical Therapist license (compact license must be in hand at time of submission)
- Minimum 1 year of skilled home health experience required
- Basic Cardiac Life Support (BCLS) certification required
- OASIS competency required
- COVID-19 primary vaccination series plus one booster required
- Must have reliable personal transportation
- Locals are accepted; candidates within a 50–75 mile radius are considered local
Preferred Experience / Skills
- Experience with EPIC Remote Client for home health charting
- Familiarity with Rover mobile documentation application
- Ability to manage time effectively across a geographically distributed patient caseload
