Contract | Full-Time | Days | Monday–Friday, 8-hour shifts | 13-Week Assignment | Onsite
Key Responsibilities
- Provide skilled home health nursing care to patients within the Sussex County, DE coverage area, including the Lewes, DE region
- Conduct and complete OASIS assessments in accordance with home health regulatory requirements
- Participate in on-call rotation approximately 2 times per month (maximum 3 per month), responding to true clinical issues during designated on-call hours
- Perform phone triage and patient home visits as needed during on-call shifts
- Participate in a weekend rotation of approximately 1 weekend every 4 weeks, with scheduling flexibility available
- Adhere to a blackout period for system transition from September 8, 2026 through October 31, 2026
Required Qualifications
- Active Delaware (DE) Registered Nurse (RN) license required
- Prior home health nursing experience required
- OASIS proficiency required
- Non-resident DE applicants should anticipate approximately 5 weeks for credentialing, including in-person fingerprinting in Delaware
- Must live at least 50 miles from the facility to be eligible for travel rates
Preferred Experience / Skills
- Experience with Epic EMR
- Familiarity with home health regulatory and documentation standards
- Comfort with independent patient management across a broad geographic coverage area
