Key Responsibilities

• Coordinate care and services for assigned patients across an episode of illness or treatment in collaboration with the patient, family, physician, patient care team, and payors
• Utilize advanced clinical skills to facilitate assessment, planning, intervention, and evaluation in accordance with the nursing process
• Evaluate and analyze quality and utilization cases, including admission/discharge criteria, observation and short stays, readmissions within 30 days, and Medicare and diagnosis-related criteria
• Promote effective utilization of resources by applying current clinical knowledge and awareness of community services, assuming a leadership role within the patient care team
• Advocate for patients and families throughout the entire episode of care, maintaining availability as a communication resource among providers and monitoring services rendered
• Initiate appropriate clinical pathways based on admission diagnosis and serve as a resource for utilization management; may be assigned to the Emergency Department

Required Qualifications

• At least one year of recent Case Management experience in an acute care setting
• Current California RN Licensure
• Current CPR certification

Preferred Experience / Skills

• Experience working in or alongside an Emergency Department setting
• Familiarity with Medicare criteria, diagnosis-related groups, and utilization management processes
• Strong communication and interdisciplinary collaboration skills across multidisciplinary care teams
