Key Responsibilities

• Provide physical therapy services to patients across an assigned multi-city territory in the greater Richmond and surrounding areas of Northern California
• Conduct evaluations, re-evaluations, OASIS assessments (Start of Care, Recertification, Resumption of Care, Discharge), and routine visits in a home health setting
• Manage a full patient caseload with an expectation of at least 27 points per week under a guarantee arrangement
• Travel outside the primary service area and provide weekend coverage as needed to meet weekly visit expectations
• Complete all visit documentation, patient and physician communications, and drive time as part of each approved visit
• Provide care to Covid-positive and Covid PUI patients as required

Required Qualifications

• Current California Physical Therapist (PT) License
• Current BLS Certification
• Valid Driver's License, current auto insurance, and current vehicle registration
• Up-to-date health clearances required: TB (annual), MMR, Varicella, HepB, TDap (within 10 years), Flu (annual)
• Must pass a pre-assignment drug screen and physical
• Must be willing to travel across multiple service areas and work weekends when needed

Preferred Experience / Skills

• Prior home health physical therapy experience
• Familiarity with OASIS documentation and home health visit workflows
• Experience managing a high-volume, geographically dispersed patient caseload
