Key Responsibilities

• Perform and document comprehensive patient assessments, incorporating culture, age, developmental level, risk factors, and presenting complaints in accordance with RN scope of practice and facility policies
• Develop and implement individualized care plans in coordination with patients, families, and the inter-professional team, adjusting interventions based on ongoing assessment and response to therapies
• Recognize, respond to, and communicate changes in patient condition; initiate actions to alleviate patient stress and discomfort
• Facilitate admission, transfer, and discharge activities, including timely execution of discharge orders, discharge teaching, and coordination with Case Management
• Delegate tasks appropriately and monitor accuracy and completeness of patient care activities performed by other team members
• Actively participate in unit-based quality improvement activities including patient satisfaction, fall prevention, pressure ulcer prevention, CAUTI reduction, and documentation auditing

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of Kentucky
• Basic Life Support (BLS) certification
• Associate Degree in Nursing (ADN or ASN) required
• Ability to communicate and collaborate effectively with all members of the care team
• Proficiency with or ability to be trained on relevant computer systems and applications

Preferred Experience / Skills

• Previous clinical nursing experience in a hospital or acute care setting
• Bachelor of Science in Nursing (BSN)
