Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Evaluate, develop, and implement individualized treatment plans based on patient needs
• Document patient progress and maintain accurate clinical records in accordance with facility requirements
• Collaborate with interdisciplinary team members to ensure continuity and quality of patient care
• Complete all required facility-specific documentation and credentialing steps prior to assignment start

Required Qualifications

• Active Physical Therapist license required; compact license accepted at start but permanent Delaware license must be obtained promptly
• Experience in an outpatient physical therapy setting
• Must meet contract QA credentialing requirements prior to start
• Candidates who have worked at this facility in a permanent or PRN capacity within the last year are not eligible for this assignment

Preferred Experience / Skills

• Prior travel therapy experience in an outpatient setting
• Strong documentation skills and ability to adapt to facility-specific EMR or charting systems
• Ability to work independently while maintaining high standards of patient care
