Key Responsibilities

• Perform and document comprehensive patient assessments, incorporating culture, age, developmental level, and risk factors in accordance with RN scope of practice and facility policies
• Develop and implement individualized plans of care in coordination with patients, families, and the inter-professional team, adjusting interventions based on ongoing assessment and response to treatment
• Recognize, respond to, and communicate changes in patient condition; initiate appropriate actions to address patient stress, discomfort, and safety concerns
• Delegate tasks appropriately and monitor team members to ensure accuracy and completeness of delegated patient care activities
• Facilitate admission, transfer, and discharge processes including timely execution of discharge orders, patient and family education, and coordination with Case Management
• Actively participate in unit-based quality improvement activities including patient satisfaction, fall prevention, pressure ulcer prevention, CAUTI reduction, and documentation auditing

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of Kentucky
• Basic Life Support (BLS) certification
• Associate Degree in Nursing (ADN or ASN) required
• Ability to communicate and collaborate effectively with all members of the care team
• Proficiency with or ability to be trained on relevant computer systems and clinical applications
• Ability to assess age-specific patient safety issues and implement appropriate safety measures

Preferred Experience / Skills

• Previous nursing experience in a Medical Surgical or Telemetry setting
• Bachelor of Science in Nursing (BSN)
