Key Responsibilities

• Provide direct patient care to residents in a long-term care/skilled nursing facility setting
• Administer medications, treatments, and nursing interventions in accordance with the plan of care
• Monitor and document patient status, reporting changes to the supervising nurse or physician as appropriate
• Collaborate with interdisciplinary care teams to support resident health and well-being
• Wear a facility-issued or agency photo badge and present a valid state or federal issued photo ID upon entry to the facility at all times

Required Qualifications

• Active and unrestricted LPN license in the state of Vermont
• Every weekend availability is required for this assignment
• All requested time off must be approved prior to contract finalization
• Must have reliable personal transportation — public transportation in this area is limited and unreliable
• Must have completed the original or updated COVID-19 vaccine series, or be able to present a documented religious and/or medical exemption from your employer
• Local candidates only

Preferred Experience / Skills

• Prior experience in a long-term care or skilled nursing facility environment
• Familiarity with electronic health records (EHR) systems commonly used in SNF settings
