Key Responsibilities
- Provide direct patient care in a long-term care/skilled nursing facility setting
- Administer medications and treatments in accordance with physician orders and facility protocols
- Monitor and document patient health status, reporting changes to the supervising RN or physician
- Collaborate with interdisciplinary care teams to support individualized patient care plans
- Maintain valid state LPN/LVN licensure and adhere to all facility compliance requirements
Required Qualifications
- Active and unrestricted LPN or LVN license in the state of New Mexico
- Must present a valid state or federal issued photo ID upon arrival at the facility and wear a photo badge at all times while on assignment
- Must have completed the original or updated COVID-19 vaccine series, or be able to present a documented religious and/or medical exemption from their employer
- Must comply with any additional vaccine or health requirements mandated by applicable law
Preferred Experience / Skills
- Prior experience in a long-term care or skilled nursing facility environment
- Familiarity with electronic health records (EHR) systems used in LTC/SNF settings
