As a key member of our care coordination team, you will play a vital role in ensuring that our patients receive seamless and high-quality care throughout their entire healthcare journey. **About the Role:** As a Registered Nurse - Nurse Care Coordinator, you will be responsible for coordinating patient care, facilitating communication between healthcare providers, and ensuring that patients receive the necessary resources and support to achieve optimal health outcomes.
