We are seeking a compassionate and skilled Licensed Vocational Nurse/Licensed Practical Nurse to join our Long-Term Care/Snife team. As an LVN/LPN, you will play a vital role in providing high-quality patient care and supporting the overall well-being of our residents.

Key Responsibilities:

• Conduct patient assessments, develop individualized care plans, and implement evidence-based interventions to promote optimal health outcomes • Administer medications, perform wound care, and provide basic medical care as needed • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate patient care • Educate patients and families on disease management, self-care techniques, and hospital policies • Document patient information accurately and maintain confidentiality • Participate in quality improvement initiatives and contribute to a culture of excellence

Requirements:

• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of [insert state] • Minimum 1 year of experience in long-term care or acute care setting • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment and prioritize tasks as needed • Basic life support (BLS) certification
