Job Summary:

We are seeking a compassionate and skilled Licensed Vocational Nurse/Licensed Practical Nurse to join our team in a Long-Term Care/Snife facility. As an LVN/LPN, you will provide high-quality patient care, support the healthcare team, and contribute to a positive and inclusive environment for our residents.

Key Responsibilities:

• Assess, plan, coordinate, and implement patient care, including developing and revising care plans as needed • Administer medications, treatments, and procedures according to facility policies and physician orders • Provide wound care, dressings, and other basic nursing skills as required • Monitor vital signs, report changes to the healthcare team, and document patient information accurately • Educate patients and families on health, wellness, and disease management • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers • Maintain patient confidentiality, respect individual rights, and adhere to facility policies and procedures • Participate in quality improvement initiatives and contribute to a culture of safety and excellence

Requirements:

• Current licensure as an LVN/LPN in the state of [insert state] • Number of years of experience in long-term care or similar healthcare setting • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment and prioritize tasks as needed • Basic life support certification (BLS) and willingness to obtain advanced life support certification (ALS) as required • High school diploma or equivalent required; associate's or bachelor's degree preferred
