Key Responsibilities

• Provide direct patient care in a Long-Term Care / Skilled Nursing Facility (LTC/SNF) setting
• Administer medications, treatments, and nursing interventions as directed by the care plan
• Monitor and document patient conditions, reporting changes to the supervising nurse or physician
• Maintain compliance with facility policies, state regulations, and nursing standards of practice
• Be available to cover shifts on a rotating, as-needed basis across multiple facility locations

Required Qualifications

• Active, unrestricted Minnesota LPN or LVN license (no temporary or pending licenses accepted)
• Experience in a Long-Term Care or Skilled Nursing Facility setting
• Ability to provide a minimum of 2 hours notice prior to shift start for any cancellations
• Willingness and ability to cover multiple locations within the designated service area

Preferred Experience / Skills

• Prior experience working PRN or Per Diem in an LTC/SNF environment
• Strong adaptability and ability to integrate quickly into varying facility workflows and teams
