We are seeking a compassionate and skilled Licensed Practical/Vocational Nurse (LVN/LPN) to join our team in a Long-Term Care/Snafi facility. As an LVN/LPN, you will play a vital role in providing high-quality patient care to our residents.

Job Type: Full-time

Responsibilities:

• Assess, plan, implement, and evaluate patient care plans to promote optimal health and well-being • Administer medications, treatments, and therapies as prescribed by physicians • Conduct vital signs, take medical histories, and perform other basic nursing skills • Collaborate with interdisciplinary teams to coordinate patient care • Educate patients, families, and caregivers on health promotion, disease prevention, and management of chronic conditions • Maintain accurate and up-to-date patient records and reports • Participate in quality improvement initiatives to enhance patient outcomes and facility operations

Requirements:
