**Licensed Vocational Nurse (LVN/LPN) - Long Term Care/Senior Living (LTC/SNF)** **Location:** Saint Albans City, VT **Shift:** Nights **Job Type:** Part-time/Temporary **Required Qualifications:** • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of VT • Experience working in a Long Term Care or Senior Living facility • Strong assessment and critical thinking skills • Ability to work independently and as part of a team • Reliable transportation (personal vehicle) required for on-call shifts **Key Responsibilities:** • Provide high-quality patient care, including administering medications and treatments • Conduct thorough assessments of patients' physical and emotional needs • Develop and implement individualized care plans to meet patients' unique needs • Collaborate with interdisciplinary teams to coordinate patient care • Maintain accurate and up-to-date patient records • Follow facility policies and procedures to ensure a safe and healthy environment for patients **Note:** This position is available for local candidates only. On-call shifts may be required, and occasional weekend shifts may be necessary. A reliable vehicle is required for on-call shifts, and Uber/Taxi services are not guaranteed in the area.
