Registered Nurse LTC/SNF
Job Title: Registered Nurse - LTC/SNF Farmington, ME
Schedule: Full-time, Days
Key Responsibilities: • Provide high-quality patient care in a long-term care setting, ensuring optimal health and well-being • Assess, plan, implement, and evaluate patient care plans, collaborating with interdisciplinary teams as needed • Develop and maintain positive relationships with patients, families, and healthcare professionals • Monitor and report patient conditions, identifying areas for improvement and implementing changes as necessary • Educate patients and families on health promotion, disease prevention, and management strategies
Required Qualifications: • Current RN licensure in the state of ME • 2+ years of experience in LTC/SNF or related setting • BLS/CPR certification • Ability to lift, transfer, and position patients as needed
Preferred Experience / Skills: • Experience with electronic medical records and documentation software • Knowledge of gerontological and medical-surgical nursing principles • Ability to communicate effectively with patients, families, and healthcare teams
