LVN / LPN LTC/SNF

Job Title: LVN / LPN - LTC/SNF

Location: Farmington, ME

Schedule: Days

Key Responsibilities:

• Provide high-quality patient care in a long-term care and skilled nursing facility setting • Develop and implement individualized care plans to meet the unique needs of residents • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes • Monitor and report changes in patient condition to ensure timely interventions and prevent complications • Educate patients and families on health promotion, disease prevention, and management of chronic conditions

Required Qualifications:

• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of Maine • Basic Life Support (BLS) certification • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced, dynamic environment • Compassionate and empathetic approach to patient care

Preferred Experience / Skills:

• Previous experience working in long-term care or skilled nursing facilities • Knowledge of gerontological nursing principles and practices • Experience with electronic medical records (EMRs) and other clinical software systems • Bilingual or multilingual skills (not required but preferred)
