Therapist – Physical Therapist

Job Title: Physical Therapist
Location: Mauston, WI
Schedule / Job Type: Full-time, Days

Key Responsibilities:

• Assess and treat patients with movement dysfunction or pain to enhance their mobility and daily functioning.
• Develop and implement individualized treatment plans aimed at pain management, fall prevention, and recovery.
• Document patient evaluations, progress notes, discharges, and other relevant care activities.
• Communicate effectively with patients, healthcare providers, insurance companies, and rehabilitation staff.
• Assist in maintaining a clean and safe treatment environment, including cleaning therapy rooms and performing light housekeeping tasks.
• Support discharge planning and patient outcome improvement efforts.

Required Qualifications:

• Valid state licensure as a Physical Therapist.
• Excellent clinical assessment and treatment skills.
• Strong documentation and communication abilities.
• Ability to work independently and as part of a multidisciplinary team.

Preferred Experience / Skills:

• Experience in outpatient or rehabilitation settings.
• Familiarity with insurance approval processes and documentation.
• Ability to assist with general cleaning and facility maintenance tasks.
