Key Responsibilities
• Provide skilled physical therapy services to patients in a short-term rehabilitation and skilled nursing facility
• Assess patient needs and develop personalized treatment plans
• Implement therapy interventions to improve mobility, strength, and function
• Monitor and document patient progress in accordance with facility protocols
• Collaborate with multidisciplinary team to ensure optimal patient care
• May provide coverage at a nearby facility within walking distance as needed

Required Qualifications
• Valid state licensure as a Physical Therapist
• Prior experience in a skilled nursing facility (SNF experience preferred)
• Strong clinical assessment and documentation skills
• Excellent communication and interpersonal skills

Preferred Experience / Skills
• Experience working in a post-acute or rehabilitation setting
• Ability to adapt therapy plans to diverse patient populations
