Key Responsibilities

• Evaluate patients' physical therapy needs and develop individualized treatment plans.
• Administer appropriate therapy treatments in accordance with agency policies and professional standards.
• Re-evaluate and adjust treatment plans based on patient progress.
• Communicate treatment activities and patient progress with interdisciplinary team members.
• Document evaluations, treatment goals, and patient progress thoroughly.
• Educate patients and family members on treatment procedures and rationale.

Required Qualifications

• Bachelor’s degree in Physical Therapy.
• Licensed as a Physical Therapist in Vermont and New Hampshire or hold an unencumbered compact license.
• At least one year of experience as a PT in a home healthcare environment.

Preferred Experience / Skills

• Experience working in home health setting preferred but not required.
• Strong communication and documentation skills.
• Ability to work independently with a focus on patient-centered care.
