Job Title

Key Responsibilities
• Assist licensed therapists in providing patient care within the scope of practice
• Conduct assessments and implement treatment plans for patients with orthopedic, geriatrics, and medical surgical needs
• Educate patients and their families on home exercise programs and proper use of assistive devices
• Monitor and record patient progress, vital signs, and responses to treatment
• Ensure safety and proper use of equipment, including gait belts, walkers, and adaptive devices
• Maintain accurate documentation using electronic medical records (EPIC)

Required Qualifications
• Current state license as a Physical Therapy Assistant
• Minimum of 2 years of experience in a healthcare setting, preferably in home health or outpatient therapy
• Knowledge of cardiopulmonary, geriatrics, orthopedics, and medical surgical conditions
• Strong assessment, critical thinking, and communication skills
• Ability to work collaboratively with the therapy team and patients

Preferred Experience / Skills
• Previous home care experience is highly desired
• Familiarity with EPIC charting system
• Ability to demonstrate organizational skills, efficiency, and flexibility in a dynamic environment
