Key Responsibilities
- Provide skilled physical therapy services to patients in a Skilled Nursing Facility (SNF).
- Assess patient needs, develop treatment plans, and implement therapy interventions.
- Monitor patient progress and adjust treatments accordingly.
- Collaborate with healthcare team to ensure optimal patient care.
- Document treatment plans and patient progress accurately.
- Comply with all regulatory and safety standards.
Required Qualifications
- Minimum of 1 year of professional SNF experience.
- Valid state licensure as a Physical Therapist.
- Demonstrated knowledge of rehabilitation techniques and protocols.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
Preferred Experience / Skills
- Experience working in a fast-paced SNF environment.
- Bilingual skills or additional certifications are a plus.
