Key Responsibilities
• Provide skilled physical therapy services to adult patients in a home health setting.
• Conduct assessments and develop individualized care plans based on patient needs.
• Manage a caseload of 5-6 clients per day, completing therapy documentation efficiently.
• Navigate various electronic medical records (EMRs) fluently and accurately.
• Collaborate with an interdisciplinary team to ensure quality patient care.
• Flexibly accommodate holiday schedules and weekend rotations as needed.
Required Qualifications
• Valid Physical Therapist license in the state of Vermont.
• Prior travel experience with at least 2 assignments preferred.
• Minimum of 3 years of clinical experience in home health care.
• Strong clinical/assessment skills.
• Proficiency with EMR systems.
• Ability to reliably travel within assigned areas using personal or rental vehicle.
Preferred Experience / Skills
• Experience with OASIS documentation and home health protocols.
• Comfortable working with diverse adult patient populations with complex needs.
