Key Responsibilities
- Provide specialized physical therapy services to patients in a skilled nursing facility setting.
- Assess patient needs and develop individualized treatment plans.
- Implement therapy programs to improve patient mobility, function, and quality of life.
- Collaborate with nursing staff and other healthcare professionals to ensure comprehensive patient care.
- Maintain accurate documentation of patient progress and treatment outcomes.
- Ensure compliance with all applicable healthcare regulations and facility policies.
Required Qualifications
- Valid state licensure as a Physical Therapist.
- Proven experience working in a skilled nursing or long-term care environment preferred.
- Strong communication and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary team.
- Knowledge of relevant healthcare regulations and safety standards.
Preferred Experience / Skills
- Prior experience in a SNF setting.
- Familiarity with electronic health record (EHR) systems.
