Key Responsibilities

• Provide specialized physical therapy services to patients in a skilled nursing facility setting.
• Assess patient needs and develop individualized treatment plans.
• Implement therapy programs to improve patient mobility, function, and quality of life.
• Collaborate with nursing staff and other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate documentation of patient progress and treatment outcomes.
• Ensure compliance with all applicable healthcare regulations and facility policies.

Required Qualifications

• Valid state licensure as a Physical Therapist.
• Proven experience working in a skilled nursing or long-term care environment preferred.
• Strong communication and interpersonal skills.
• Ability to work collaboratively within a multidisciplinary team.
• Knowledge of relevant healthcare regulations and safety standards.

Preferred Experience / Skills

• Prior experience in a SNF setting.
• Familiarity with electronic health record (EHR) systems.

• A 10% placement fee applies.
• No weekend rotations; schedule is Monday through Friday.
