Key Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility.
• Develop and implement individualized treatment plans to assist in patient recovery.
• Evaluate patient progress and adjust treatments accordingly to optimize outcomes.
• Collaborate with multidisciplinary team members to ensure comprehensive patient care.
• Document patient assessments, progress notes, and treatment details accurately and timely.
• Participate in weekend rotation as part of the scheduled shifts.

Required Qualifications

• Valid state licensure as a Physical Therapist.
• Experience working in a skilled nursing or rehabilitation environment preferred.
• Strong patient assessment and treatment planning skills.
• Excellent communication and teamwork abilities.
• Ability to work effectively in a fast-paced healthcare setting.

Preferred Experience / Skills

• Prior experience in long-term care or post-hospital rehabilitation.
• Knowledge of Medicare, Medicaid, and private insurance billing and documentation.
