Schedule / Job Type
- Full-time, Days shifts, Monday through Friday with weekend rotation
Key Responsibilities
- Evaluate and develop individualized treatment plans for patients recovering from illness or injury
- Provide rehabilitative therapies to enhance mobility, strength, and overall function
- Collaborate with nursing staff and physicians to ensure optimal patient care
- Document patient progress and adjust treatment plans as needed
- Maintain compliance with regulatory and facility-specific requirements
Required Qualifications
- Valid state license or certification as a Physical Therapist
- Proven experience in rehabilitation or skilled nursing settings preferred
- Strong communication and interpersonal skills
- Ability to work effectively within a team environment
Preferred Experience / Skills
- Experience with post-hospital and short-term rehabilitation patients
- Knowledge of Medicare, Medicaid, and private insurance processes
- Prior experience in a skilled nursing facility
