• Full-time, Days shifts, Monday through Friday with weekend rotation

Key Responsibilities

• Evaluate and develop individualized treatment plans for patients recovering from illness or injury
• Provide rehabilitative therapies to enhance mobility, strength, and overall function
• Collaborate with nursing staff and physicians to ensure optimal patient care
• Document patient progress and adjust treatment plans as needed
• Maintain compliance with regulatory and facility-specific requirements

Required Qualifications

• Valid state license or certification as a Physical Therapist
• Proven experience in rehabilitation or skilled nursing settings preferred
• Strong communication and interpersonal skills
• Ability to work effectively within a team environment

Preferred Experience / Skills

• Experience with post-hospital and short-term rehabilitation patients
• Knowledge of Medicare, Medicaid, and private insurance processes
• Prior experience in a skilled nursing facility
