Key Responsibilities

• Assess and develop treatment plans for patients in skilled nursing facilities (SNFs).
• Provide personalized physical therapy interventions to improve patient mobility and function.
• Document patient progress and update treatment plans accordingly.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Educate patients and families on treatment protocols and injury prevention.

Required Qualifications

• Valid state license or certification as a Physical Therapist.
• Minimum of 1 year of professional experience working in a Skilled Nursing Facility (SNF).
• Strong understanding of physical therapy principles and practices.
• Excellent communication and documentation skills.
• Ability to work collaboratively within a healthcare team.

Preferred Experience / Skills

• Experience working in a long-term care or SNF environment.
• Knowledge of current therapeutic techniques and equipment.
• Bilingual abilities are a plus.
