Key Responsibilities

• Provide physical therapy services to patients in a Skilled Nursing Facility setting.
• Develop and implement personalized treatment plans based on patient needs.
• Monitor patient progress and adjust therapy programs accordingly.
• Collaborate with healthcare team members to ensure optimal patient outcomes.
• Maintain accurate documentation of patient treatments and progress.

Required Qualifications

• Valid state license as a Physical Therapist.
• Ability to work the scheduled days (Monday through Friday).
• Experience working in a Skilled Nursing Facility (SNF) preferred.

Preferred Experience / Skills

• Previous experience in a SNF setting is a plus.
• Strong communication and teamwork skills.
