Key Responsibilities
- Provide physical therapy services to patients in a Skilled Nursing Facility setting.
- Develop and implement personalized treatment plans based on patient needs.
- Monitor patient progress and adjust therapy programs accordingly.
- Collaborate with healthcare team members to ensure optimal patient outcomes.
- Maintain accurate documentation of patient treatments and progress.
Required Qualifications
- Valid state license as a Physical Therapist.
- Ability to work the scheduled days (Monday through Friday).
- Experience working in a Skilled Nursing Facility (SNF) preferred.
Preferred Experience / Skills
- Previous experience in a SNF setting is a plus.
- Strong communication and teamwork skills.
