- Full-time, Days shift, 8-hour shifts, 5 days a week
Key Responsibilities
- Provide physical therapy services to patients in a skilled nursing facility (SNF) setting
- Develop and implement individualized treatment plans for patients
- Monitor and document patient progress and adjust treatments accordingly
- Collaborate with healthcare team members to ensure optimal patient care
- Ensure compliance with all regulatory and safety standards
Required Qualifications
- Valid state license as a Physical Therapist
- Proven experience in a SNF or similar long-term care environment
- Strong patient assessment and treatment planning skills
- Excellent communication and teamwork abilities
Preferred Experience / Skills
- Prior experience working with diverse patient populations in a rehabilitation setting
- Knowledge of current physical therapy techniques and technologies
