Key Responsibilities
- Provide skilled physical therapy services to patients in a skilled nursing facility (SNF) setting.
- Develop and implement individualized treatment plans based on patient needs.
- Monitor patient progress and modify therapy plans accordingly.
- Collaborate with healthcare team members to ensure quality patient care.
- Document patient assessments, progress, and treatment sessions accurately.
Required Qualifications
- Must have at least 1 year of professional SNF experience.
- Valid license or certification to practice as a Physical Therapist.
- Strong knowledge of physical therapy techniques and patient care protocols.
- Excellent communication and interpersonal skills.
Preferred Experience / Skills
- Experience working in a fast-paced SNF environment.
- Ability to work independently and as part of a team.
