Key Responsibilities

• Provide skilled physical therapy services to patients in a skilled nursing facility (SNF) setting.
• Develop and implement individualized treatment plans based on patient needs.
• Monitor patient progress and modify therapy plans accordingly.
• Collaborate with healthcare team members to ensure quality patient care.
• Document patient assessments, progress, and treatment sessions accurately.

Required Qualifications

• Must have at least 1 year of professional SNF experience.
• Valid license or certification to practice as a Physical Therapist.
• Strong knowledge of physical therapy techniques and patient care protocols.
• Excellent communication and interpersonal skills.

Preferred Experience / Skills

• Experience working in a fast-paced SNF environment.
• Ability to work independently and as part of a team.
