Schedule / Job Type
- Full-time, nights shifts, 12-hour shifts, approximately 3 shifts per week
Key Responsibilities
- Provide skilled nursing care to residents in a LTC/SNF setting during night shifts
- Administer medications and treatments as prescribed
- Observe and document patient conditions and report any changes
- Collaborate with the healthcare team to ensure quality resident care
- Adhere to all facility policies, procedures, and safety protocols
Required Qualifications
- Active MN LVN or LPN license
- Valid MN SNRA registration
- Previous experience in LTC or SNF preferred
- Knowledge of PCC electronic medical records system
- Ability to work independently during night shifts
Preferred Experience / Skills
- Experience working with elderly or chronically ill patients
- Strong communication and teamwork skills
