Key Responsibilities
• Provide outpatient physical therapy services to patients in a skilled nursing facility
• Develop and implement individualized treatment plans based on patient assessments
• Monitor patient progress and adjust therapies accordingly
• Document patient care activities accurately and timely
• Collaborate with healthcare team members to ensure optimal patient outcomes

Required Qualifications
• Valid and active State Physical Therapist license
• At least 1 year of professional SNF (skilled nursing facility) experience
• Strong knowledge of physical therapy techniques and modalities
• Excellent communication and patient care skills

Preferred Experience / Skills
• Prior experience working in a skilled nursing setting
• Ability to adapt treatment plans to diverse patient needs
