Key Responsibilities

• Provide skilled physical therapy services to home health patients in Salem, OR, and surrounding areas.
• Conduct patient assessments and develop individualized treatment plans.
• Document patient progress accurately using remote documentation tools such as EPIC, Rover, and Oasis.
• Collaborate with the healthcare team to ensure quality patient care.
• Travel within a designated radius to visit up to six patients per day, optimizing route planning to minimize drive time.
• Maintain compliance with facility and state regulations, including COVID vaccination requirements.

Required Qualifications

• Valid state license to practice as a Physical Therapist.
• BCLS certification.
• Minimum of 1 year of skilled home health experience.
• Reliable personal transportation.
• Ability to work independently and manage a mobile caseload.

Preferred Experience / Skills

• Experience using EPIC, Rover, and Oasis for documentation.
• Prior experience in home health setting.
• Familiarity with rural or outpatient therapy environments.
